Engineer Crushed to Death at Work

On August 1, 2024, two companies were fined after a Health and Safety Executive (HSE) investigation found they were responsible for a fatal workplace accident that claimed the life of Russell Hartley, a 48-year-old father of three, in February 2020.

The incident occurred at a recycling facility in Bow, East London, England, where Premier Engineering Projects Ltd contracted Hartley to replace a Trisomat screen, also known as a “flip-flop,” a machine used to sort waste by size. 

During the operation, the screen toppled forward while being moved with a telehandler and a crane supplied by M&M Mobile Crane Hire Ltd, resulting in Hartley’s death. Another worker standing on the screen was thrown off but escaped serious injury.

The HSE investigation revealed significant safety breaches. Both companies failed to plan, supervise, and execute the work safely, and the risk assessments were found to be unsuitable and insufficient. 

Premier Engineering Projects Ltd and M&M Mobile Crane Hire Ltd pleaded guilty to breaching the Health and Safety at Work Act 1974. Premier Engineering Projects was fined $35,840 and paid $11,875 in costs, while M&M Mobile Crane Hire Ltd was fined $61,440 and paid $12,160 in costs.

Russell’s wife, Debbie Hartleyshared her profound sorrow and disbelief, describing Russell as central to their lives. She remembered him as humorous and exceptionally kind, expressing that she often waits for him to return, unable to fully accept his absence due to not having the chance to say goodbye. Debbie also mentioned that Russell was dedicated to ensuring the happiness of his family, and she is committed to honoring his wishes by continuing to strive for their happiness.

An HSE inspector, Mark Slater, highlighted that the risks of handling and moving large equipment are entirely predictable. He emphasized that such work requires the highest level of attention and precaution.

The fines highlight the severe consequences of failing to ensure workplace safety.
