The “A League of Their Own” star underwent a lower deep-plane facelift in January 2026, a procedure that lifts tissue under the facial muscles to avoid the skin looking too tight. Rather than transform her entire face, O’Donnell focused only on rejuvenating her lower face and neck.
She first went public with the news in a Substack essay titled “Decisions,” published on May 25, 2026, followed by before-and-after photos posted to Instagram on May 27. Speaking with E! News on the Tonys red carpet, O’Donnell explained why she chose to share the personal decision so openly.
“Authenticity is the goal in these days and times,” she said. “And I think all that matters is truth and love, and so I wanted to be truthful and say all the complicated emotions I had about it.”
She added that she’d rather control the narrative than wait for a tabloid to publish a “Gotcha!” headline. O’Donnell also joked about the cost, saying the procedure was “more expensive than any car I ever bought, but I can’t drive around in my face.” Asked whether she would consider additional work, she answered simply: “No, I don’t think so.”

The former co-host of “The View” tied her decision directly to her dramatic weight loss. O’Donnell, who has Type 2 diabetes, began taking the GLP-1 medication Mounjaro in late 2022 and has lost more than 50 pounds over the past three years. Mounjaro is also used by stars like Amy Schumer and Vanessa Williams.
“I’m on Mounjaro over the last three years, I have diabetes too, and I lost over 50 pounds, and that was responsible for a lot of the extra skin that I had around my face. And there were two lines that made me look sad,” she explained.
In her Substack essay, O’Donnell described her face as having taken on a “haunted” look after the weight loss, and admitted her face had begun “melting.” Since moving to Howth, Ireland, in 2025, locals had repeatedly mistaken her resting expression for sadness. “In Ireland, people would say, ‘Are you upset, darling? What’s the matter, love?’ And I’m like, ‘That’s just my face. I am not upset; it’s just how I look,'” she shared.
An Honest Conversation With Clay
O’Donnell also opened up about the conversations she had with her 13-year-old, Clay, before going through with the procedure. “My little autistic 13-year-old said to me, ‘If you do it, I would not be able to respect you ever again,'” she recalled. “And that put me off it for a few months, but then when I went and did it in January, I came home 10 days later, and they never noticed.”
In her essay reflecting on the decision, the comedian acknowledged she had once been firmly against facelifts. “I used to feel very strongly about facelifts. Not casually — morally. I had assigned myself as head of all women who would never — ever,” she wrote. “I thought it was a betrayal. Of feminism. Of aging. Of our team of women worldwide. And then I lost 50 pounds…”
Ultimately, O’Donnell said she wanted to model bodily autonomy for her child. “If I’m teaching Clay anything, it can’t be that my body belongs to an idea either. Even a good idea. Even feminism,” she wrote. The procedure, she added, made her look like “a slightly more well-rested emotionally stable version of me.”
