The strange, boxy bump beneath President Donald Trump’s suit jacket reappeared on Wednesday, September 16, 2026, this time during a campaign trip to North Carolina. The bulge, clearly visible through the back of the 80-year-old president’s suit, showed up in a short video posted to an Instagram story by White House aide Margo Martin, who filmed him taking questions from reporters in front of Air Force One.
The shape pushed the fabric of his jacket into an unnatural wrinkle and stayed fixed in place while he spoke. The sighting was at least the fourth of its kind in recent months, and it came in the middle of a trip Trump made to boost Republican Senate nominee Michael Whatley.
The White House Answers With an Insult
The White House has not said what is under the suit. Asked about it, White House spokesman Davis Ingle emailed the Daily Beast a statement on Thursday, September 17, that went after the news organization rather than the bulge.
“Only the glue-sniffing degenerates at the failing Daily Beast would obsessively push baseless conspiracy theories about President Trump, and that’s because they suffer from a severe, crippling disease known as Trump Derangement Syndrome that has completely rotted their peanut-sized brains,” Ingle said.
He continued: “President Trump’s sharpness, unmatched energy, relentless work ethic, and historic accessibility stand in stark contrast to what we saw during the last administration when Democrats and the legacy media intentionally covered up Joe Biden’s serious mental and physical decline from the American people.”
From “60 Minutes” to a Long Island Rally
The shape first drew notice in April, when viewers of Trump’s “60 Minutes” interview fixated on an unusual outline under his jacket. Weeks later, on Memorial Day, May 25, video from the wreath-laying ceremony at the Tomb of the Unknown Soldier at Arlington National Cemetery — where the president appeared alongside Vice President JD Vance and Defense Secretary Pete Hegseth — set off another round of online commentary about a bulge in his clothing.
On Friday, August 14, then-White House Press Secretary Karoline Leavitt, who has since left the post, posted footage on X from a rally on Long Island. In it, Trump steps down from the stage toward the crowd, the back of his suit jacket pushed outward by something rigid that does not shift as he walks.
Guesses, Jokes and a Biographer’s Pushback
With no explanation from the administration, the internet has filled the vacuum.
“It looks like he has some sort of contraption back there,” independent journalist Aaron Rupar wrote on X after the Long Island video circulated. One widely shared post in August likened the bump to a shoplifted Thanksgiving turkey. The recurring claim that Trump wears adult diapers is not new, and it has a prominent skeptic: a biographer of the president told the podcast “Inside Trump’s Head” that there is no basis to support it.
Scrutiny of the President’s Health
The bulge is only the latest entry on a growing list. Trump, the oldest person ever inaugurated as president, has been photographed with bruised hands, swollen ankles and a neck rash, and critics have accused him of dragging or tapping his right leg as he walks. He has fallen asleep at public events, and he appeared unable to keep his eyes open at a September 11 attacks memorial.
Questions about his mental sharpness have trailed him just as closely. He has boasted more than once about his performance on a dementia screening exam that he describes as a measure of intelligence. In July, a White House teleprompter operator accused of using advance knowledge of Trump’s speeches to place bets on Kalshi was placed on unpaid leave and later left his government job. At the Long Island rally, Trump seemed to lose the name of Saritha Komatireddy, the candidate he had endorsed for New York attorney general, telling the crowd he had met her for the first time that day before calling out from the podium to locate her.
Trump has answered the scrutiny with declarations of vigor. A checkup at Walter Reed National Military Medical Center on May 26, which he called his “6 month physical,” kept him there for more than three hours, and he posted on Truth Social afterward that “everything” had “checked out PERFECTLY.” Administration officials have insisted he remains in excellent health.
None of that has answered the question the cameras keep raising. As of September 25, the White House still had not said what the box-like shape is, leaving a mystery that resurfaces every time a photographer catches the president from behind.
